  4. Whereas, Charlotte Barker-Atwell and Kenneth Atwell, are the
  5. parent and natural guardians of the following child(ren):
  6. Alison, Bryan, and Chester
  7.  
  8. I designate Doreen Atwell of Chattanooga, Tennessee to act as
  9. guardian of the minor child(ren) stated above upon my incapacity
  10. to so act.
  11.  
  12. Should Doreen Atwell be unable or unwilling to serve, I nominate
  13. Louise Barker of Jackson, Mississippi to act as the guardian of
  14. the minor children in the place of Doreen Atwell.
  15.  
  16. Upon my disability, the designated guardian shall have the
  17. following authority:
  18.  
  19. a) residential custody of the minor child(ren);
  20.  
  21. b) to approve medical treatment of any kind or type or to
  22. disapprove the same within the bounds of the law;
  23.  
  24. c) to designate schooling for the minor children, and access to
  25. any and all of their educational records;
  26.  
  27. d) to generally act in loco parentis.
  28.  
  29. In the event that I am the custodian of any property for the minor
  30. children under the Uniform Transfer to Minor's Act, or the Uniform
  31. Gifts to Minors Act or similar statute,  I designate the guardian
  32. or successor guardian to act as custodian for all such custodial
  33. property.
  34.  
  35. In the event that formal legal proceedings are commenced to
  36. establish a guardian for the child, it is my desire that the
  37. guardians mentioned herein have priority in appointment.
  38.  
  39. The failure to list an individual as a guardian or successor
  40. guardian is intentional.
